I travel there all the time and always get my Visa taken care of by a
local travel agency. They will charge your about $60USD for a 1 entry
(you can enter China once) or $90USD for 2 entries.

You will need to bring a passport photo and your passport.

(6) months validity left before expiration and at least one blank page
left in it.

But that's if you have already made it to asia. Otherwise, most places that sell plane tickets to China can arrange an agent to get the visa for you.
